Gave Amd another shot and I'm having issues, again
. After running crysis 2 for 10-15 minutes. The game crashes and goes to bsod. I get error 116. I tried drivers 12.6, 12.4, 12.3, 12.1 and their respective caps. The cards are loading 99% in Eyefinity which was hard to achieve in the first place. Drivers 12.6 has poor 3dmark11 results but anything prior to this its ok. I have updated my mb to the most current stable version. Temps are ~70 for the top two cards, and 60 for the bottom card. Fraps says I'm getting ~60-80fps in the game. I did run the game in Eyefinity with two cards and it worked fine though with frames ~40-50. I'm running Crysis 2 @ 3240x1920, high settings, no dx11, and all ccc settings set to application default. My cards are in pcie slot #1, #3, and #5. Cards are still running stock clocks btw.
I've done some research and I have a few theories.
1) My cards are need a bump in voltage at their pcie. I have posted this issue on the EVGA forums and I'm waiting for a reply as I'm unsure where to increase the voltage for a Z77 (let alone, an lga1155 board).
2) Driver instability. I've done this one extensively so I leaning on dropping this as a culprit. Though, sometimes the cards only load to 70-80% and after driver reinstall, they load 98-99%.
3) The 3rd card is defective. This card was bought off ebay and apparently its 6 months old. I never had a bsod with the first two cards which are a year old. I'm going to pull the card later today and test the two first cards in Eyefinity.
Any other help or theories are welcome

I did loaded the card only 
. Well, no issues running tri-fire. Fraps says I'm doing +100 fps and the cards are loading ~50-60% which is typical for 1080. 3dmark11 has the three cards loading 99% so I'm sure driver 12.3 is stable again. Sigh
