If I delve into the gubbins and take the heatsink off am I likely to find poor contact with the GPU?

What model do you have?

Even my x-fire 6850 were not that hot at load. You def. need to reseat the heatsink and re-apply the TIM.
Dont put in too much TIM, also, because it will makes the things worse.

EDIT : Saw that you have an XFX, I got one in my other computer. I can confirm that those temps are def. not normal.

How you getting on with that Sandisk Extreme, BTW, I keep looking at the 120 GB version- it's around £70 on Amazon.

I'd like to know what paste they use as stock before putting anything new on, it can't be all that bad.
I think it just needs a better heatsink/fan, this one goes at over 60% just with two monitors on idle, but I wonder why this one is hot and most of the reviews of the XFX 6850 are about 20c cooler.
Believe me, it's bad. Pulling the heatsink off without reapplying thermal material is a big no-no. Replace it with some MX-2, MX-3 or MX-4 and you'll definitely notice a difference.
You could also be having case temp/airflow issues as well.

Is the fan even spinning?, or is it running at absolute minimum RPM or something? + What others have said about the re-apply. That smells more like no fan than bad TIMM application unless ofcourse there is no conctact.

It's pretty good, no issues. About 6 months old.
I would rather go with Samsung 830 if you find it on sale at this point though, better controller and a little faster.

It's bad because that introduces the air pockets. The paste isn't there to fill in "air pockets" but to fill in the microscopic gaps between surfaces. I would repaste the card and see if your temps go down. The fan could be slow or malfunctioning as well, it could be a defective card or the heatsink could be filled with dust. I would not get a new cooler for that card, it's too old, just invest in a new card instead if this one is broken. I assume it is out of warranty, yes?

Hmm, but I don't see the reasoning. Even better to leave the original paste on because it's already worked into any microscopic fissures in the surface, I would have thought.

You kind of undid that when you pulled the heatsink off...

Well, the paste is there for the microscopic gaps, fissures and scratches, and also - judging by the way people lap there allegedly wonky CPU's to fill larger gaps, all of which will be filled with air, air being made of small enough molecules to fill any microscopic gaps.

Unless there's some reason that air is repelled from processor packaging surfaces?
