Bad PSU ?

I am not quite sure what is causing my system crashes but what I do know is when it crashes my system does not want to start again, All it does is puts my CPU fan to a 100% and after 4 seconds restarts and does the same thing again until I turn the PSU off, So I ran OCCT to check my voltages and here is what we get, Also is that 12V sensor wrong is there something really wrong with my PSU?

It says in Your Computer section Power Supply Modded? What do you mean modded? What brand is it? If i were you i would buy a power supply tester of ebay around $5 and it will tell you if something is wrong, Software is not accurate, i also believe you can check inside the BIOS under Hardware Monitor?

The 120 ml fan broke so I put in a faster 80 ml fan in
And BIOS voltages aren't loaded so their clean but it still jumps around from 11.98 to 12.02
