I have a small problem. Small part of an instrument i use broke. The part is made out of acrylic.

On the sketchup drawing i made, the part thickness is set to 5 mm but i believe its 1/4" which translates to 5.3 mm.
The hole in almost the middle of the part is for M4 threaded insert. I am not sure what diameter it should have but I made is slightly less than 4 mm so the insert had enough material to melt and fuze with. Any correction/discussion on this matter would be welcome.
In short: I need few parts like the one I broke made (see the attachments). Is anyone on this forum able to make those and if so - how much wold that cost me?
