Bitfenix Survivor that is pretty much the worst cooling case I have ever tested or the discontinued

Or you could look at the Corsair C70 or Antec Lanboy Air which both have carrying handles and sell for $60-$70 more.
Personally, I've owned quite a few CM cases and almost always replace all the stock fans immediately. Their stock case fans are anemic at best. They could use the R4's but that would necessitate an integrated fan controller.
It's stamped plastic. Once you have the the mold for that piece it's like stamping any other piece, handles or no handles. Unless there's additional effort or cost in creating integrated carrying handles during the manufacturing process I don't know about.

You're right about stock fans, even on a Lian Li case I replaced all the stock fans for higher static pressure ones. Personally I think manufacturers should sell 2 versions of a case, one with fans and one without, with the case without fans being a little cheaper.
