I have 100+ clients and if was manage everyone website i'd NEVER EVER EVER have time to do some actual designing, company's like to update there websites regularly and so they should, but its not my job to do that unless they're paying me a monthly fee for the pleasure, and even then its a twice a month max job...
Most clients want to have control over their site and most hosting sites give the option to change text pictures through a very rudimentary html editor. but if you're using lots of php and js this isn't really a viable option.
What i usually suggest is that they employ someone in-house (part time) or 3rd party to manage the site.
Small companies want the whole thing, but i still suggest they set up the hosting for their own tax purposes.
For charging I always give a set price, if i know its gonna take me 4 weeks full time to do then i'd charge them around 4k, but what i do is ask for 50% upfront and 50% once completed and happy.
I think as you're starting out, 20-30 (I work in Euro/GBP) per hour is reasonable, bare in mind there are many cheaper quite decent web builders out there now... but you're not just offering the design, its the whole advice, service and aftercare too.
Its always tough to fix your prices, lump sum estimates are what the clients wants but you need to quantify your time (we've all done 8+ hour sessions without realizing...)
