For a 4.5GHz OC you will need to add more voltage.
CPU-Z reports 1.26v when running 4.5GHz... Am I supposed to add even more voltage?

Fill in your system specs so we can see what cooling you have and what PSU you have, many things can cause this.

I read the CPU goes vcore hunger after 4.4-4.5GHz.

I was doing 4.5GHz at 1.260v onload, stable for a day then wants 1.296v onload, lasted 2 days and wants more. I ended up tweak to 4.2GHz with fixed normal VID vcore and dynamic vcore offset -0.050. Runs at 1.164v stable for a while now. Big difference. But still every CPU and mobo are different, there are Maximus V Formula club at the asus ROG forum, should try read some posts there for reference to your OC. Definitely not the GPU's fault wink.gif
