Graphics Cards Unstable from Factory?

My friend and I recently had a little dispute over weather or not graphics cards with factory overclocks could come unstable from the factory (too little vcore, too much core clock, etc)

My reasoning was that they could not possibly QC every single card, and even if they did, bad cards could slip through the cracks sometimes.

He insisted that they stress tested every single card in a hot box to make sure they are stable. He said that unstable cards are a 1 in 1,000,000 occurrence and 9 times out of 10, the problem is with the end user making a mistake.

Who is correct?

Not sure, but I'm sure it has happened before. I wouldn't think it is common or on purpose though.
