i5 Overclock 3570k - P95 OC Error

When overclocking my 3570K I've been experiencing issues that result from only one thread specifically and one core.
After around 10+ minutes of P95 'Core #2' unsuspectingly drops from 100% load to 0-25%. The errors are replicated throughout my higher overclocks specifically on this core.

The " Fatal hardware errors " in the logs also represent this core specifically when the test fails.
In short my question is whether or not this is an actual hardware failure (dead/dying core), or just a sign I have to tweak more of my voltages again?
