What do i do with the 4 pin header on a water cooling build? stick it on the back as exhaust? Split into 3 signals and run it to the fans on my top radiator?

Do you mean a 4pin fan header?

If so, that is just a normal 3pin + PWM signal. You can use the PWM option to control things like PWM fans or PWM variable speed pumps which can be tied to thermally throttle fans/pumps. May also be run to some fan controllers like the Sunbeam Rheosmarts to control the speed of non PWM fans, etc.
