With this game being so old, will it even play on modern PC's with a modern OS (like Windows 7)?

They bundle Dosbox with it to emulate DOS, works perfectly on my Windows 7 system.

You can always just rip the duke3d.grp out of the main folder and use it in EDuke32. Add polymer and HRP textures to get this.

I'd recommend eDuke regardless, makes the game play more like a modern game, but it doesn't make the game any easier, The Birth, the level of nightmares 0.o

He must not have mouse aiming enabled. It's definitely in the options though.
