What is wrong with my GPU ?

So i got my rig a few month ago and nothing bad has happened so far until earlier today. I was playing guild wars 2 when suddenly the whole screen turned blue and the monitor showed "No Signal" . When that happened the 1st thing that came into my mind was to replug the HDMI cable and i did that but nothing happened so i turned off my comp and replug the cable and went back into guild wars and it happened again. This time i got so pissed that i spammed my ESC button and everything when back to normal except that guild wars started to lag alot . It went from 60fps to 20fps and stayed around 20 . I did check my Temps , it was around 47c idle ( i think it this high because the fan blades are dusty and i dont really dare to clean it )and around 60c+ in game .
I got no idea whats going on with my computer and if anyone can help that would be great.

Tried restarting my comp after the FPS drop and it went back to normal , I also notice that my USB 3 pro was not connected properly to my MOBO , did that cause the problem?

I just think the gpu crashed, it didn't get fixed after a reboot?

It actually did that 3 times , the 1st two times I did the procidure of turning off the comp and replugging the cable but when it happen on the 3rd time I raged and spammed the Ecs key and everything went back to normal but the game had low Fps. But after I restarted my comp everything went well , at least for now. If its just the Gpu crashing should I reinstall drivers or just leave it and see what happens?

My Gpu's fan is also really dusty how should I clean them?

Yes I would second a driver reinstall. Its not likely to be a dusty fan, but its always good to keep it clean . You could use one of those air cans to clean the heatsink/fan.

Thanks for the reply , guess i would reinstall the drivers , as for the dust , the GPU fans are really dustly because its placed on the floor and i'm unsure of the proper way of cleaning out the dust without spoiling anything.

It just happened again , this time spamming the Esc key didnt work but it did bring me to a screen that look similar to this ( http://img862.imageshack.us/img862/7892/img0192bt.jpg ) got the picture through google. but this time i notice that i could hear the game sound still going on when i press Skills and such.

Did it happen post a driver wipe ? I'd reccomend using Driver Fusion and doing a total wipe of your driver and re installing them.

Thanks , guess i'll download Driver fusion to do a wipe then . Really hope nothings wrong with the card .

Dammit!the problem came back again.after installing the new driver (12.11 beta) nothing went wrong for About a day or 2 till windows suddenly had to reformat the computer back to the time when it was performing normal again. Then windowsmsaid that i didnt have a gpu driver and i went on and downloaded 12.10 this time, now the problem got alittle worst . Even when im not on guild wars ( which was the program i notice this started happenimg) like when im on dota 2 or just browseing the web the problem of having no signal then a message of mode not supported will appear on my monitor which is an lcd tv. I got no idea whats wrong with the comp , i did build it myself so it could be anything. Could it be That my psu isnt supplying suffient amount of electricity to my gpu?

This problem is getting Really annoying , I cant play any games and even when im on the browser ill always have the fear that it would happen. If anyone can Help it would be awesome!

The power supply is good enough if it's functioning at its peak, but is it? That's the question. Do you have a spare PSU to test?

Try earlier drivers, then you know 12.11 beta is your problem.

5% slower and no faults would be better, IMHO.
