I don't think a slide-out MB tray will be necessary with this design. It will just add complexity and cost. For a small case like this, steel may be a better choice as well - not sure...

I like the pedestal idea for adding rads/drives while still keeping a small footprint.

To me the ultimate mITX awesome case would be like my Lian Li PC-Q25B (in that it has 5 HDD racks) with more room in the front to run a 120mm / 140mm rad and 3 expansion slots (to fit one of those monster 3 slot cards)

What I'm thinking of is something like this - though I don't think it would happen.
Get rid of the flex bays and mount the PSU next to the board, as there is need for clearance for a long GPU anyway, have a thin 240 P/P on top and bottom and have enough clearance behind the motherboard for a slim optical drive and the ssds/HD
2 minute drawing:

if you make the feet and top swappable you can go normal or reverse atx, and all you need is a separate back panel to make it horizontal HTPC style
You could even chop off the top and bottom rads into "pedestals" which could be for drive bays or rads or nothing (for minimal air coolers)
One thing I would like to see though is an uninterrupted front panel, i.e. you sell different sizes depending on the number of pedestals. I hate the join on the front with cases with pedestals.

Interesting. I'm not sure how well that PSU placement will work though...

OMG - Yes please!!! (I'm still saving for ST10, but this is great - I promise, I will save harder
This are my wishes (I would use my for HTPC/NAS):
- at least 4 HDD + 1 or 2 SSD (maby they will make ITX board with 6 sata ports)
- 1 dual slot graphic card
- room for 240 (280) radiator (and small pump and reservoir)....but I would probaby use H80/H100 kraken
- normal modular PSU
- it has to be elegant industrial design - just like other CL cases, but without DVD drive in front....I dont mind it DVD is excluded
- you can put USB port and power button on side panel
- in ideal case - it could stand in "portrat" or "landscape" position
in "landscape" position - width would be same or less than typical width of audio AMP
"landscape" position width = "portrait" position hight
ok, once you make that box - make sure that you can stack another box on it - in that other box (pedestal) make room for extra HDDs larger radioators and what not - then I could use it as render machine for 3DS Max
and, if there is any possible way to put mATX board - that would be just....perfect!!!

yeah although it fits it gives you minimal space for the wires to come out. Might need a bit more room and of course a 90" power cable in the back of it. Maybe if it was rotated 90 degrees it would help.
Essentially when you have to cater to long GPUs it means there is a bunch of wasted space there as the mitx board is not so deep

I don't think that long GPU's will be a problem. With the Flex-Bays, you can either have a long GPU + RAD for a gaming rig or a RAID Controller + HDD cages for a server/NAS.

I could see a case with the features of the sm8 just compressed into a smaller size fit for the ITX mobo. Like stren's idea, except mount the psu in the back and at the bottom so you have room in the front for possibly a 120/140 or 240/280 rad. that could leave room for a short drive bay (if you were not feeling the behind the mobo tray idea), or a pump/res in the bottom front under the gpu and in front of the psu. we are talking about caselabs so i would not be expecting super tiny.
