I shutdown my rig and turned it on and it happened again frown.gif Changed the xHCI mode to auto again and all is well so far. Do I really need the xHCI mode set to auto smart? Should I just disable pre boot driver or just leave it be?
no you don't need to have it set to smart auto, I think most folks use auto. my system boots 4-5 seconds faster on smart auto, so that's why I left it there. smart auto used to be "default" but the more recent bios versions set "auto" as default. you can try disabling the pre boot driver all together, and if that works for you then great. might just check to make sure you can use your keyboard prior to OS launching, otherwise it will be a drag when you need to get into bios. Im just wondering what the root of the issue is, and it's stumped me. could be a bad board. you might trying gigabyte customer support, and see if they have any ideas.

How do I disable the pre boot driver all together?

The fix that's been working so far is your first suggestion: plug keyboard into other usb. I just changed where my mouse is plugged in and it works so far. Do you know why it does that in other USB ports?

I remember reading in the manual of my p9x79 pro that it said to only use USB 3.0 ports for external drives or high bandwidth devices.

I recently noticed that my keyboard (plugged in to USB 2 port) wasn't working at all during boot so anytime I got an overclock error or otherwise press F1 to resume message I couldn't do anything other than restart. Couldn't get in to bios either. Tried all different USB 2.0 ports but no difference. Realised eventually that my mouse plugged into a USB 3.0 hub on my monitor was the cause of the issues.

As for CPU fan not spinning, something is very wrong there. Are you using any low noise adapters (LNA) for the fan? These are simply inline resistors to drop the voltage to the fan (speed is proportional to voltage for a DC fan). If you try to drive them too slowly then they can fail to start. Less likely to occur on a 4-pin PWM controlled fan than a dumb 3-pin voltage controlled fan though.

Is the fan definitely plugge into the CPU_FAN socket on the motherboard?

Can you take screenshots of the bios settings and post them as suggested before? May help diagnose the problem

So the splash screen issue definitely involves USB ports and devices. Since I switched USB ports I haven't had the issue but a friend of mine who has the same set up is still having the same problem. Is this USB stuff causing problems happen all the time or is this something that I should RMA?

I started a new thread with the CPU fan issue, come and chime in: http://www.overclock.net/t/1351629/phanteks-phtc14pe-stops-spinning
