Stereo left/right getting mixed up issues

For some time now, I'm having audio issues while playing games/watching films. The audio channels seem to switch from left to right and vice versa. For example; when a car crashes left, I hear it crash right, occasionally. Sometimes the channels are switched, sometimes they're not. Weird. I'm assuming a software issue with the receiver (Harman Kardon HK3940). It still happens when the PlayStation 3 is on so it's either my receiver and/or TV (Panasonic TX-P42G25) I'm not sure whether it happens while watching regular TV or not, but it surely does happen regularly while playing games/watching films.

Does anyone have any idea? Is it a known issue?

I also have an HK 3490, with the TV's optical out going to the receiver.

This same thing was happening to me, and I found a workaround for when it happens. Simply switch inputs on the TV back and forth, and it should be normal again.

As in, switch from HDMI 1 to HDMI 2, and then back to HDMI 1. (The input names are just examples.)
