EK Reservoir Leaking

I built a water cooling system for my computer a couple months back. And it worked perfectly, until about a week ago I checked inside my case, and find my reservoir (a EK-MultiOption RES X3 250) is leaking. I drain the loop, take out the reservoir, and find this.

I was using pure distilled water with a silver kill coil, and that's it. The hole with all the damage around it was the outlet. What I'm thinking is that it cracked (you can see it, at the bottom of the top left hole), and the water slowly dripped out and damaged the exterior on the bottom.

Any ideas what happened, and if it was my mistake or a manufacturer defect?

Yeah. And you can't see it too well in the pics, but those chunks taken out of the plastic around the hole are about half a centimeter deep.
