New 660TI - Graphics Error in Counter-Strike:Source

Hello all,

I built a new machine this weekend, and I am having problems with Counter-Strike:Source in particular. The game runs fine, great performance, etc. Then randomly the screen freezes/turns black, sound repeats for a few seconds, and then either:
(a) the game continues playing, with poor performance, or
(b) the game continues but I cannot see what is going on, it just shows the desktop (I can still here the sounds and stuff though)

It also crashes the views in Google Chrome (they turn gray)

It's very strange because last night I played with no issues whatsoever, and the only thing that would have changed some windows updates were installed.

The machine has the following (ask if more details are needed):
  • intel i5 3370K
  • Gigabyte GA-B75M-D3P
  • EVGA GTX 660TI

What I have tried:
  • Rebooting
  • New Beta drivers (313.96)
  • Older (random) driver (306.23)
  • Disabling on of the displays
  • Turning down graphics settings

I would really appreciate some advice. I'm not sure what else to try.

Thanks!

Do you have any OC applied? Whats your cpu and ram clocks?

The game keeps running slow because of the GPU throttling way down to about 300mhz.

What power supply do you have btw?

Nope, no OCs. Everything is factory default settings.
500W Corsair PSU.
