Ouch. Have you thought of affixing the acrylic to some sort of support/bracket to create your "box", or whatever you're going for? I created something semi-similar, a desk-case, in which I screwed the acrylic directly to the support:

after destroying that sheet of acrylic i'm waiting to have the funds for

called every glass place around here and they don't do bending at all.
strange thing is they are all also more expensive for the same sheet thickness/size vs Lowes
lowes wants $55 and everyone else is pretty much $65 - $75.

have you thought of not bending and just cutting seperate sheets to screw down? there is no way you will get a good bend on a piece that large. only a plastic fabrication shop can do that. there is one here in houston i think that can fab anything out of acrylic. good luck man.
