HWMonitor isn't always accurate but I think it's time for a new PSU. You don't have a brand or model listed in your sig rig so am I right in assuming it's just a generic PSU?

Yep, just some generic PoS like half the rest of my system, and I knew this going into the "build". But I really did not expect it to be that bad. Once I find my multimeter, I'm gunna see if it is really running at ~2v

It software reading data off some inaccurate sensor on the motherboard. The system wouldn't boot the voltage was 40% off.

Use a multimeter or oscilloscope to get a proper read.
Once again...

It wouldn't boot, but HWmonitor is pretty good at finding things wrong with PSUs. It probably is off by quite a bit, just not that much.

Not true at all.... HWmonitor just reads the data off the motherboard sensor. These sensors are known to be inaccurate.

There is no magic that monitoring software can do to correct inaccurate data.

You can get a good enough PSU that isn't a POS for only $30.
