According to cpu-z mine is running 1.24v right now at 4.5 ghz. I might need to bump it up just a hair, but so far it's ok.

offset +.070?! I think you're overloading the voltage which is causing instability. What's your VID for 4.5?

+0.07 is nothing. in fact most cpu's can take at least 3x that safely

Sorry to say but you got unlucky. 4.5GHz @ ~1.35 V is unfortunate. Mine requires 1.27 V for same overclock and I would consider it an average chip. Try reapplying thermal paste and reseat the water block. Have you thought about delidding?
