RealTemp reports the percentage of time your CPU is in the C0 state. Most CPUs will use at least the C1 low power state when they have nothing to do. This significantly decreases power consumption and your core temperature when lightly loaded or idle. Your Windows High Performance profile is screwed up. It is preventing your CPU from using C1.

When C1 is working correctly, the RealTemp load value and the Task Manager will be similar.
