Turn on LLC. Always use LLC when trying to overclock. LLC protects you from the effects of vdroop, which could very well be what is bringing your system down. I'd try to get 3.8 at the same settings with LLC before doing anything else, in fact.

You can go higher on the voltage than where you are. Denebs can take up to 1.55v with little or no risk of damage as long as you keep them cool. A max of 1.45v is more what you'd look for if you were running a Thuban.

Not all chips are equal. Some 965's will reach 3.8 and even 4.0 comically easy. Others, you have to do some coaxing to get them there.

Enabled CPU LLC. Seems stable now with 19 multi making 3.8 GHz at 1.4 V. Seems that V droop was pretty bad....Thought the mobo I had was a good one for OC but I guess not.
In the BIOS desciption for LLC is suggests there are levels of LLC ranging from 1 to 5. However, I only see the option to have LLC enabled, disabled or auto. When enabled, no additional options appear. Picture below of what I am talking about.





Also, should I be using CPU/NB LLC as well, as long as my NB is OCed?

Yes use CPU/NB LLC if you OC the NB.

That MOBO isn't great for overclocking, the M5A99X-EVO would have been a better choice from that lineup and general price range. My fiance is running my old one right now with a 4.25GHz OC on her 960T (Thuban).

The descriptions in the BIOS might not be 100% accurate for that board - I'm assuming they just hacked up a BIOS from another board and didn't pay mind to the descriptions. I think you can only set it on or off with that board IIRC - my little cousin has the same MOBO.
