Good (extra) low profile options?

I have 40mm of height clearance, and pcb components / ram sticks in close proximity (itx); and I need to cool this AMD A8-3870k - I'm currently using a Noctua NH-L9a and I hate to say it but I'm not impressed. The CPU idles at 47*C and at 100% load the system quickly overheats. It doesn't seem to be making good contact and there's nothing I can do about that with the supplied mounting kit. The heatsink doesn't have any vertical play but I can wiggle it back and forth. I don't need overclocking headroom - but this is a home server and I'd prefer it to be stable to some extent. I've reseated the heatsink twice, and replaced the compound application both times. I may be able to attach a 240mm rad to the rear of this case with fans in push/pull but I'd prefer to avoid that as it would make it rather unpractical - that and I'd have to cannabalize the pump from an all-in one unit and I'd really prefer to stay far from those.

Anyways - suggestions on a sub 40mm height cooler - keeping in mind that 40mm means that whatever is at the 40mm mark will be about a sheet of paper or two away from the nearest object

Well I'm pretty sure you could fit a radiator internally I myself have some tiny dual 40mm rads in use and they fit pretty much anything and certainly cool enough for just that proc. (with 2 you should get better than that 47 you're currently getting)
