HD6870 constantly running at 100% fan speed.

I recently reformatted my PC, and re-installed the drivers. Everything was fine, until a few days later, my GPU (HD6870) decided to run at 100% fan speed. I've tried manually adjusting the fan speed with CCC and MSI afterburner. It'll just display that the fan is running at the speed I selected, but by the noise I can tell the fan is still running 100%. I also tried drivesweeper and re-installed the latest drivers. Physically removing the card itself and putting it back didn't work too. It's been running this loudly for days now, even in idle.

When idle, core clock and memory clock go down to 300MHz.
Temps have been around ~30-40C, so I don't know why it is running at 100%.

Any ideas on what to do?

I would roll back to the previous driver release......idk what version you installed but for me 13.4 is most stable. The betas give me artifacts on desktop. I'm running 7870 Tahiti. And fan speed goes up to max on the betas just like on yours until I apply custom profile in afterburner.
