Exactly. As long as BO2 has a "raw mouse input" checkbox, you won't need the MarkC fix -- but it's not much trouble to apply anyways.

If you want to measure LOD, the best way is to put the mouse on top of CDs and let the sensor look through the middle. Each CD is 1.2mm thick so it provides a rough estimate of how high the mouse will track. However, I think you only 'need' to use the tape trick if you find that when you lift the mouse 'normally' it doesn't stop tracking or jitters/tracks in a strange way. If you don't notice any problems in practice when lifting the mouse up, there's no need to fix it.

I haven't seen anyone play without lifting their mouse, to be honest. Some people I see play on really high sens using just their wrist/fingers but I see them lift occasionally to recenter the mouse. I play on really low sens using my entire arm and so I'm constantly picking up the mouse and sweeping it around the mouse pad.

If you don't lift your mouse though, clearly you don't have to worry about it!

No -- maybe a centimeter?
