It sounds like you want open world like Skyrim, but world-changing decisions and a dynamic story like Dragon Age and Mass Effect. Thankfully, such a game will attempt to provide the best of both worlds: Dragon Age: Inquisition, which comes out next year. Not to mention, it will still have the industry leading character development found in those games too. If you haven't played those games, you should, perhaps starting with DA:O and some good mods like Combat Tweaks, Improved Atmosphere, Circles be Gone, Skip the Fade, and Grey Wardens of Ferelden.

The Witcher 3 might try too, since it's going to be open world, but I seriously doubt they'll provide a good open world experience on their first try, and The Witcher games don't have nearly as much story or world variation as Dragon Age. They also only have one class/playstyle and only a tiny fraction of gameplay diversity, but who knows if TW3 will change that.

 

I don't particularly want to kill important characters right now and bug out my save. I just thought, say if the whole world was much bigger you could have more realistic meetings with Ulfric, in a huge chamber surrounded by guards, and go for a crazy assassination attempt...

 
But yo're right about what I want, DA:I caught my eye and I'm definitely going to check it out. DA:O is something I'll try (after Skyrim) as well as Oblivion to see what they're like. 

 

For my "kill anyone" idea I wonder if it's possible to set up huge faction trees with rules on what happens if members are killed...

Oblivion is very similar to Skyrim, except the open world is less lively and less detailed. It does have better quests at least. Morrowind is much more impressive than Oblivion; the open world is truly unique, though even more static and lifeless with greatly simplified dungeons, but again it has stronger quests than Skyrim.
