OCZ Vertex 2 BSoD - need help with smart info/finding the issue

Hey guys.
Ive had this 60gb OCZ Vertex 2 for a few years now and its been a good experience so far.
But recently ive started to get some BSoD's - and after these BSoD's my BIOS will no longer detect my SSD.
Doing a few reboots lets my BIOS re-detect the SSD and boot into windows just fine - it will happen again later though, sometimes it goes for a couple days, sometimes only a few hours.
Since my BIOS wont detect my SSD after these BSoD's i think its safe to say its an issue with the SSD.
The question is, is it fixable? biggrin.gif

Ive highlighted the line i think means that my SSD is just getting old and dying.
But truth be told i dont understand half of what is showed on the Smart Data.

When working is the drive listed properly in the BIOS?
Have you tried re-seating power/data cables? Or tried new ones?

Yes when it works its listed properly in BIOS.

Ive also tried reseating the cables, switching cables and tried to switch sata port as well - no luck
