Could you use the drain port as an Inlet/outlet

You could, but I would highly advise against it.

The input/output ports send the coolant though the rad in a long u shape, the length of the rad and back again.

Using the drain port as an inlet/outlet would cut that in half, yielding only half of the cooling benefits of the rad.

If you had a LOT of Head Pressure in your loop (dual D5 in serial) and split the input into two lines one for each side, I imagine it'd work (using the full area of the radiator) but you'd have a slow flow through the radiator in comparison to the rest of the loop..

Edit:

Would the slow-flow through the radiator increase performance - as it'd have longer in the radiator..?

You could use a Y Fitting for it i guess.
