Monitor goes black for 2-3 seconds

Playing a game and now my middle monitor will sometimes go black and it carys over to sesktop (lasts about 2 seconds). Then after it comes back it looks grainy for a 2-3 seconds.

While in BF4 I was getting graphics that made the ground look like a grid, like the textures weren't loading fast enough after this happened.

I am using a club3d MST hub which splits a single displayport output into 3 displayport outputs for eyefinity. I am wondering if this is an issue with MST hub, bad displayport cable, or possibly a GPU failure?

I did randomly get a BSOD last night but I restarted my PC and didn't get any pop up as to why. I looked and my front 200mm case fan wasn't working so I fixed it suspecting I got a GPU overheat or something.

Any help is greatly appreciated.

Ok I switched the plugs arround on my MST hub to see if it was that, and my middle monitor is still getting grainy lines across it while web browsing. At this point I think its either the monitor or the displayport cable.
