Black screen of death, HD7950 GPU Issues?

Hello to all!
To a dear friend of mine is a strange thing happened, very strange ....
He is an owner of Sapphire HD 7950 Dual X boost with the latest bios. Today he has overclocked 1.2 V @ 1150 with 1350Mhz on the ram.
After 5 minutes of play, the pc crashes and shuts off. Restart the PC after the boot logo and the monitor goes black and the computer freezes.

Tests performed: Formatting, start with the second bios, reinstalling drivers, bios reflash, test on another pc and always get the same result.
Without drivers the pc starts normally, with drivers get crash on Both PCs
What has happened to this board?

could just be an bad GFX bios flash

make sure he has plugged in the 6pin + 8pin pci-e

also if you can what is his PSU model

We have reflashed the bios, all compatible version.
The PSU (600W OCZ modXtreme pro) is working with other graphics card
This GPU has been tested on another PC with the same results
