R8 280x or GTX770 would be your best choices.
The 280X being a little faster due to the res you play at.
Last 6 months on the driver side have been better then nvidias ones since they have really been pushing there driver department in the last year.
I had a 5830 and can agree with you back then the drivers were lacking.
There are are few things that can be improved but with what your doing they wont effect you (multiscreen and 1600p+ frame pacing)

Just good luck finding one for a decent price so youd be better off getting a 770 if the market there is jacked up like it is where I live with 280X selling for $500-600

You don't NEED to upgrade your CPU to run a GTX 780, but it is recommended. Just trust me when I say that for gaming, a new i5/i7 quad or an FX 8 core are far superior to your current CPU. There will be games that you're going to run that you may not have the highest framerates simply because a Phenom II X6 cannot drive a GTX 780 hard enough.

Using a GTX 770 or 760, it should be fine, but on a GTX 780, I think you will cross the line of diminishing returns with your current CPU.
