I can't say I've ever used the one Fast suggested, but the UBCD4 WIN boots you into what looks like windows XP. Then you go to your all programs, passwords, and there should be NTPassword edit. all GUI based.

Boot off of a hirens usb or cd into the windows xp option. I am using 15.2
open the HBCDMenu.exe and find the password renew program. or simply browse the programs directory yourself.
Click (Select a target) and browse for the windows directory. This should be C:\Windows.
once this is done you should be able to renew an existing password using the task on the left, (renew existing user password) you can also create a new admin user if you like.
Type in the password you like in the required fields, note* password cannot be blank. just for the sake of simplicity i recommend using the number 1 as the password.
This should change the password and allow you into the login.
Restart the machine out of the hirens windows xp and allow it to boot up to vista.
enter in the password that you had created for the user and see if it works.

HIrens or NT Password Recovery Tool (which is basically just in Hirens)

I am a field engineer in the IT field, have recovered over 300 passwords with a Hiren's disc.

I have not used NTPWEdit before but from briefly looking at it it should work the same as my previous suggestion.
