Since your 5870 does the same thing, maybe not a card issue? How stable is your CPU oc? I have had some pretty crazy issues from unstable CPU oc in the past. To the point that I thought i was using a P4, but the system was not crashing completely.

Since you've used a 5850 as well and are having the same issue, I don't think another card will solve your problems.

Maybe bad memory? Try 1 stick in each slot, then another stick in each slot.

You're using a SSD, try install on another hard drive or SSD. Try different SATA ports as well. Unplug the DVD SATA cable while you're at it.

From there run the CPU stock...

If something is still off, I would tend to think it's the motherboard...

Try a different monitor and video outputs (No adapters)

That blows though you have a fast system and it's doing this.

You know what I notice you only have 4gb of ram. And is probably the reason why you have problems in the first place.

My brother was having the same problems in games like BF4. Upgraded to 8GB problem is gone. 4GB is not enough for most newer games.

Seen a bunch of posts where people with i3's and 4gb of ram were having same problem till upgrading to 8gb. Shouldn't really be any different for your i5.

He already posted earlier that his rigbuilder specs were out of date and that he is running 8GB of RAM.

As stated by everyone previously, go into troubleshooting mode.
Unplug everything not necesary to run computer and a game on it.
Run a memtest.
I have had memory do weird things to my system only when under load... Perhaps this is your case?
Sometimes memory is just incompatible with your motherboard. My gskil memory will bsod and hard freeze my amd fm2+ motherboard all day but runs fine on my intel system (what I'm typing with). And it actually passed memtest downclocked from 2133 to 1600 but did 27+ passes on memtest no errors. Still hard freeze when booting and mining.
Now that amd system currently only freezes on load (mining) with my other stick of ram (crucial). Don't have games on it yet. My problem may be too much power being pulled from the wall since I have another mining rig running off the same power extension.
It could be the power supply too.
After my OCZ ssd bricked on me during linux installation I no longer trust any of there products. So try a new power supply.
Return your CPU to full stock.
No crazy coreparking or whatever. Just set bios to load default values.
Try a new ssd with windows 7/8 on it. No extra hard drive plugged in.
If you have another system try putting it that.

Truthfully I would try power supply first since that would be less expensive then new ram or gpu.
That is if you are done messing with your current system and want to buy something.
Otherwise, play around with some of the above suggestions.

i had a couple of asus 6970's that were studder-y-ish until i chased it down to the psu.....

the only dif in this situation is that your 3d11 scores seem normal....mine were quite low

It might just be the card itself at this point. I once bought a reference 5870 and that would stutter regardless of what I tried (like in your case). Strangely enough, it worked perfectly well outside of gaming (videos or even mining), so I ended up just selling it on Ebay for a buyer who was into mining. I ended up buying two non-ref 5870s after that and it worked well for me before I switched to my 7950.

Outside of that single 5870, I've had zero problems with all my ATI/AMD cards over the years (4890, 5770CF, 5870CF, 7950).

Just make sure your issue is related to your GPU as someone mentioned. I've seen a few people who thought it was their GPU and after trying a new GPU or switching teams, still encountered the same issues.
