No not just yet. First I would take off your side panel of your case, gently touch the tubes and see if they're giving off a gentle vibration, if they're dead quiet that could indicate a pump failure.

Next it might be best to re-seat the water block and confirm that the block is making good contact with the chip/enough thermal paste is there.

Don't forget to blow the radiator clean. If it is plugged with dust it won't work well.

Even newer aio models can choke on temperatures. 140XL sports great results however keep it at that for 10 minutes and temperature is no different than any other cooler.
Water is such a good heat carrier that you won't notice a difference until it reaches equilibrium with the cpu. Then, it is up to how effective your radiator is until the cpu is cool again.

But this is right at start up. Loaded PC, started ESO, and its instantly 100c.

Zidion sounds like your H60 could be faulty. Mine did this and my temps were really high. I think when mine was faulty the temperature was really hot and the H60 tubes were really hot to the touch.
