Paste the file to into "C:\Users\Username\Documents\The Witcher 3" , then run the game, select English (if it isn't), then exit the game again without touching any graphics, open user.settings and change resolution to your monitor's resolution, V-Sync, and whichever other options that way (not through in-game options because applying them can reset the tweaks). Once you've done that - make user.settings a read-only file. You in-game video graphics options are going to several sliders at Very Low settings (Lowest actually), but in reality they higher than Ultra settings.

.To make them work, download the latest ReShade Framework 1.0.0, extract it to "The Witcher 3 Wild Hunt \bin\ x64" folder so that ReShade folder and ReShade64/32.dll files are in the same directory as "witcher3.exe". Delete any already existing d3d11.dll and dxgi.dll files along with ReShade32.dll. Then rename ReShade64.dll to dxgi.dll. Finally paste my Framework settings files into "x64\ReShade" folder and replace existing files. That should do it.
