How about your PSU. Is it old, underpowered, or a cheap one?

I had some issues with throttling a while back that I blamed on my 780. I RMA'ed it, got a replacement, and had the same issue. Come to find out it was my PSU (TX850M). I replaced the PSU with a X-850 and no longer had the problems.

Just something to look at.

I remember having odd ball issues like you mentioned before the PSU finally started really giving me problems. When it did finally start really crapping out, it wasn't able to hold over 600Mhz on my GPU.

One other thing, try and listen for your PSU fan. Mine started making struggling sounds towards the end and sounded very bad when starting up. I was able to hear it because of the case I have. You might want to take the side off of your case for a night and keep a ear out for any odd sounds coming from your PSU.
