What Shader Cache? I tried all Crimson Drivers and its changes nothing for me.

All Crimson versions enforce shader cache for The Witcher III. The files can be found under:
C:\Users\YOURNAME\AppData\Local\AMD\DxCache

Despite this, it did not reduce or resolve the issue for many when using CFX and it may have only helped single GPU's, even though it didn't need it.

Shader cache helps when the scene has objects with different shaders. Amd preloads the shaders to the cache so they dont have to load on fly the shader. But on the horse movement that the shaders doesnt change shader cache will not gonna help.

Do you have pagefile on? The modern games from my testing use a lot of pagefile. I have 12gb on my ssd.

I have what ever Pagefile is set to default. Single GPU is very smooth but its slow because only 25-30 fps. Dual GPU get 40-50 fps with drops to 30 when GPU usage hit 55% in new scenes with a lot of stutter when you run or in the house. At 1440p it was ok but 4K its not playable. From experience Witcher 3 has had the worse CFX stuttering of any games that support CFX.

I tried with and without. I still get stuttering on horse back, especially at full sprint. I had this issue on GTAV as well (but with cars lol), but AMD improved this noticeably.

Its an IO bottleneck probably. Do you have the games on ssd?

I came back to FO4 and Witcher 3 after 16.1.1 drivers, and game-play is much smoother than at the launch. I guess I should do that with every new AAA game, wait a month or 2 before playing.

Never really had problems with FreeSync, worked from day 1 for every game. The Division was a bit strange, FreeSync wasn't working, but when I alt-tab to desktop and alt-tab back to game it would work fine.

Sure do.

I have a Samsung 840 Pro SSD connected to SATAIII 6GB/s.
I tried the following:

Installed IRST.
Installed latest chipset drivers.
Disabled onboard sound.
Disabled HPET and all other none used ports/features.
Disabled all C states.
Disabled Intel link power management.
Different OS.
Different BIOS.
Different monitor and monitor output cable.
Configured game settings to medium.
Disabled HT.

PCI-E ports @ 16x 3.0 on both GPU's.
ULPS disabled.
High Performance power plan enabled in the OS.

Disabling CFX fixes the issue. I'm not the only one. Even people on X99 effected!

Stutter also happenes in AC Syndicate, GTAV, Saint Row IV, Fallout 4, Far Cry 4 and a few other open world games when "CFX is enabled". It's as if the games are loading data on the fly, causing a slowdown.

I had the same issue, it was weird but in the end it worked so I didn't think much of it.
