Try reseating your PSU cables that goes to gpu make sure they are working fine and the card itself on the pciex slot reseat.

I just did that and moved GPU to a new slot. Will post back with results.

Try manually setting PCIE to Gen2 (then later try Gen3) instead of Auto, just to test if that makes any difference.

Tried this already. No difference. EVGA has offered to RMA so I might do it. I've put in a new PCIE cable in and still no difference. I reseated all PSU cables too. Only time when GPU load goes 80%+ is during benchmarking software like Furmark and Firestrike. Other than that, the GPU just chills.

Just an update. Received replacement and almost no difference in Firestrike: http://www.3dmark.com/fs/7779438

The only thing I can think of is a clean OS install.

What drivers have you tried? I'd do some clean driver installs using DDU and test again. Doing a clean OS install wouldn't hurt, I do that every 6 months or so anyway. Whats CPU usage when gaming? Tho looking at your setup it shouldn't be a bottleneck.

I've done the clean install with DDU 8 times now, and checked clean install within Nvidia's installer too. I'm doing the clean install of the OS to Windows 10 now.

CPU usage never exceeds 75%, usually 50-60%. As I said too, only on Firestrike and Furmark does it seem to have good GPU load. At this point, it's either the OS or the motherboard or just something out of my realm...

You have a 4790k getting about 75% usage which means you are getting 6 cores loaded. Also, PS2 does only use 3 (and the third is barely used).

As for low benchmark scores, I wouldn't know what would be causing an issue there.
