This has nothing to do with tickrate. There's no latency limit on Overwatch (or it's well over 200ms), and it is set up to favor the shooter. If the other player has 200+ ping, the only ill effect from it is occasionally you get the "shot behind wall" effect, on both sides.
It's a pretty awesome system if you play with people in other countries. I've played with a friend in Australia, and you really couldn't tell since the client doesn't lag like it does in CS. The sniper rifle scoped in instantly, instead of in CS where there's a noticeable 200ms delay before anything you do happens.
I imagine quick play tries to group you with people who have a reasonable ping, which is why it only happens sometimes.
