With locations, make sure to keep an eye out for private property or secured areas... Sometimes people had submitted portals for Ingress in those types of areas, and not all of them were weeded out despite multiple reports from Ingress agents about such.
When you throw a pokéball and it misses, you do indeed lose it. In order to avoid buying more, a much wiser course of action is to visit pokéstops, whereby once you get about 20m or so from it (it seems to be about half the range of Ingress portals, and they share the same database) you then click on the pokéstop, which gets it to zoom in on your screen and pretty much fill it. At that point, swipe from one side to the other, and it should spin. Once it spins, it will shoot out "bubbles" containing items. You can usually get 3-4 Pokéballs by doing this at every stop. Recharge rate seems to be the same as Ingress portals, which is 300 seconds aka 5 minutes.
As for duplicates of Pokémon you do not want, if you pull up their screen (click on the Pokémon itself) there should be a button near the bottom labelled "Transfer". Note that this is something that cannot be reversed, and will give you cookies if I remember correctly.
