How/Where would I purchase the "Intel Performance Tuning Protection Plan"

I saw on another post about a person worried about the rather short 30-day warranty and someone from SL suggesting they could purchase
an "Intel Performance Tuning Protection Plan" from Intel on the X/K chips.

Question: If I purchase a delidded, binned chip, say a 6700k, from Silicon Lottery, how/where would I go to add the Intel Performance Tuning Protection Plan?

Pretend I am a complete idiot and tell me what steps to take.

Example:
I order the chip from you... it gets to me. is there a website or something I go to and enter my CPU serial number and then pay Intel for the warranty?
They then send me what, a receipt?
If the chip goes bad within the Intel protection plan warranty do I send the chip to Intel for a new one/refund?
Do I send it to SL? (After the 30-day warranty)

How do I know what all the plan covers and doesn't cover?

Sorry if this has been answered before, but I did try looking around for the answer first.

But this will not be good for delided CPU! Must be intact with all serial numbers visible etc.

I thought when SL delidded a CPU for you they put the lid back on?
I thought the delidding was just to replace the less than stellar Intel paste?

Well, if Intel don't know it happened....but I have never seen the results up close....

We do seal the lid back on, but from what I've heard Intel's performance tuning plan does not generally cover CPUs that have been delidded. I recommend contacting Intel themselves to get a solid answer.
