So aside from removing the changes that you did that started these problems (Samsung SSD and third monitor) and testing on a new install on a separate partition and only on one monitor, there is something else you can try.

which outlines the issues I had with W3 and games like Paragon. Turns out my 6 year old HX750 PSU was in fact kicking the bucket and wasn't providing proper power to my gpu.
As another approach to solving this (frustrating) issue you can test with another PSU. It might be a bit inconvenient to detach everything, but ruling this out with say a friends PSU might end the head-banging you've been doing to solve this.
If it doesn't then continue on with diagnostics on your SSD/HDD's etc. as those are the next most likely culprits.
