New video after updating Samsung 840 EVO firmware, using the optimisation tool. Could it be my RAM? I did the windows mem test that requires a restart and no errors were found.

Did the fw update include executing the Performance Restoration bit?

RE rams, if you added another 16GB to your original 16GB - try taking out either the newer or older pair (i.e. don't mix them) & test again

Those were added way before this issue first started. This issue started when I bought a third monitor + get the Samsung 850 EVO. Seems like its leaning towards the SSD causing trouble though.

I couldnt find the Performance restoration bit, but I believe its included in the firmware update as addressed by Samsung.

Ah I see, not really sure what to suggest at this point

RE Resource Monitor screen, on the top window if you tick "System" instead of "Wow-64.exe" will it show any breakdown of windows' processes in the 2nd window?

If its possible, revert back to 1511 (Threshold update) and your problem will go away.
The issue IS the Windows 10, no point in searching for it somewhere else. If you don't have the Windows.old already, just format the C:\ and manually install the 1511 from USB.

Wow thats a huge reddit post with so many issues in there. Going to slowly try them all one by one and see which works

So aside from removing the changes that you did that started these problems (Samsung SSD and third monitor) and testing on a new install on a separate partition and only on one monitor, there is something else you can try.

I recently solved an issue with some games that I couldn't figure out whatsoever. This is my post on the witcher 3 reddit which outlines the issues I had with W3 and games like Paragon. Turns out my 6 year old HX750 PSU was in fact kicking the bucket and wasn't providing proper power to my gpu.

As another approach to solving this (frustrating) issue you can test with another PSU. It might be a bit inconvenient to detach everything, but ruling this out with say a friends PSU might end the head-banging you've been doing to solve this.

If it doesn't then continue on with diagnostics on your SSD/HDD's etc. as those are the next most likely culprits.

OP, i have tried all possible solutions on that Reddit thread, and for some reason they worked.

But i had issues with the GPU drivers, so i went yesterday back to Windows 7. If you can, try with Windows 7.
