Whether im connected via ethernet or wifi, my internet speed is slow. My internet package is 200/20, speedtest shows ~12Mbps download. The other devices on my network run fine. The network connection status on my wifi adapter shows 650Mbps. It hasnt always been this slow, but im not sure when it started. Any ideas where i should start looking?

What are the settings on your router? How many devices? How much streaming? What are your speeds like when connected directly to your modem? Are there any switches between you and your wireless AP (access point)?

I have the netgear nighthawk ac1900 gateway, and the setting should be stock. I've only setup a password.. At the moment there are only 2 devices connected, my pc and my phone. My speeds for my pc via ethernet is ~12Mbps, and my wife's pc via ethernet is ~230Mbps. My phone speed via 2.4ghz wifi is ~90Mbps.
I installed the latest drivers earlier today. I feel it has to be something beyond the wifi adapter since it's still slow via ethernet cable. I have not tried the dongle on the other pc.
I realise you note it is the PC but testing the dongle in another PC will isolate it.
In the task manager>>Resource Monitor Network tab how many processes (with Network Activity) are running?
The PC doesn't have uTorrent or any other similar app launching at start up? uToorent by default will launch with windows - I have had to deselect this option numerous times on users PC due to it hogging the network.
In Task Manager>>Startup do you see any unusual apps?
Nothing like this in your control panel?
Do not reinstall windows, figure out what is causing the problem because if it happens again your going to be more frustrated.
Completely uninstall AND delete USB+chipset driver with device manager, reboot and install latest drivers for both. Reboot again and try the connecting and installing the adapter again.

If you have issues when using a wired connection as well.. Have you installed any software that affects the network stack or internet browsing in any way? VPN/Proxy/Firewall/Virtualization/etc.
So, we've isolated the fact that it's your PC. What are the USB settings like in the BIOS? Maybe they got flipped to legacy only? It's rare, I know, but wouldn't hurt to check at this point.
EDIT: What happens when you use an Ubuntu Live CD? We're not installing it, just using it to check throughput. We're trying to isolate software from hardware.
