Creative ?
Did you disable the motherboard's on-board audio, in the BIOS, when you added a sound card?
Its a Creative X-Fi XtremeMusic and yeah the onboard sound has been disabled for years ever since I built the machine.

The DSP series that is used the XtremeMusic sound card, Creative stopped making them a few years ago.
So I would not expect Creative to spend a lot on keeping the drivers update.
You might consider replacing the XtremeMusic with a Sound Blaster Z card.
The SB-Z has a better DAC chip and better headphone amp, then the XtremeMusic.

Have you configured all of these?

There's also some setting in Nvidia Control Panel, if you have it installed.
