It would have to be the OEM HD 6950 I bought a few months ago.

The moment I turned on my computer, the card fried itself and my board. Lost $300.

Yeah, I wasn't too happy about it at the time. It did force me to go out and get a job though. Probably a net gain for any 17 year old in retrospect. Plus I ended up with an MSI GeForce 2 Ti 64MB which is my favorite graphics card to this day. Man that thing could overclock!

Some card I used around the year 2000, maybe one of the voodoo 3dfX cards. I requested a graphics card in our family computer for my birthday and could not seem to get anything working it was always hit or miss if you were going to play a game. I believe we then updated to windows xp eventually after going from 95-98-Me/2000-95-XP. God I hated that thing . I think it came with Sonic the Headchog on CD-ROM, that was the best part .

EDIT : Actually I seem to remember it being an nvidia chip and putting it into a pentium 4 sony vaio desktop where it barely fit because of how compact it was, couldn't have been very good . . . . hahaha

radeon 9600 pro .... the seller trick me with higher vram to stash more money... the card has hard time to play many game... just wasting my money lol
the only thing it do well is quake 3... i buy in 2007 .... not realize it 4 year old already... blame my agp slot.. ill will never buy any oem pc anymore...
