What sound driver does linux use with your on-board sound? If you have any applications running sound in the background, most likely that's why it won't work. You need a card that supports hardware mixing (most creative cards support this, cheap ones too).

Find out what sound card you have and check the info at the ALSA project.

Try using OSS
Code:
quake4-smp +set s_driver oss
and if you have an app blocking your sound card it is probably the arts demon.
