The chances? Well, if you just got a hard drive, test it with scandisk and don't put anything important on it for a week or so, just to make sure its a good drive. Assuming it is, then it should last for many years. You can even still find hard drives from the 80s and stuff sold on ebay.
How does it happen? Many ways. But as long as you dont toss your hard drive out the window, give it a bath, plug and unplug it over and over just to hear it spin up, or anything stupid like that, don't worry.
You can't really lower the chances of losing a drive without not using it at all. But you CAN lower the chances of losing your data by either running two identical drives in RAID1 (two mirror images) or using a second drive to backup your files to.
