Pic included, anything look out of the ordinary 4.0ghz

Cant keep it stable on orthos for even 2 secs...
even SuperPI just shuts down after 2nd calcualtions
However does anything look out of the ordinary?
Any suggestions need to keep it stable?

looks good to me. However, I'm not very familiar with voltages for your chip maybe someone could shed light on whether they are to high. I know that 45nm aren't as tough as the 65nm when it comes to voltages.

1:1 might be holding you back on your ram.

A couple of things to look at, first what voltage did you set your RAM to. It also would be good to post a link for your RAM. Second, you probably want to bump up your NB( might be called MCH/MCP). If nsb is supposed to be FSB you may be O.K. there, and I wouldn't go any higher with vcore.

How did you get to 4GHz? Did you just jump straight from 3 to 4 and adjusted some voltages that someone else had used and posted ? Every setup is different. You need to play with volts and speeds to suit yours.

Most people would say take it a step at a time. Read some basic overclocking guides first. Get your system stable first with small overclocks. Maybe go to 3.6 with 400 FSB and find out what CPU voltage you need for this to be stable. You might have a CPU that needs more voltage than the 'norm'. Then take it up from there, using both CPU tests and Memory tests along the way.

Why did you adjust the SB voltage?

try dropping your CPU freq down from 445 to 430

okay I am starting over to 3.6 instead, is my goal to get the lowest amount of voltage needed to run it stable correct?

Correct. Higher volts = Higher temps and more wear and tear.
