nm = nanometers
It is just a measure of how small each transistor is
45nm means that the amount of transisters in a cpu die is larger - therefore more power [processing] in the same amount of space

Problem with 45nm chips [the new intel ones] is that high voltages will kill them quickly [comparitivly to the 65nm] because there is so little between each transister - only about 4 atoms i think i read some where - this means that electricity can get through the very thin barriers are damage the chips.

EDIT: the 65nm chips are the E6750, Q6600, E6600 etc, 45nm chips are the E8400, E8500, etc
