GPU is artificating, so that means it's probably very close to dying.

Have you overclocked it?

That means your video card is overclocked too high, or is overheating!

Slightly, I only raised the core to 650 mhz and shader to 1625 and the memory to 950. I didn't think that slight of an increase would do anything. I also had slight artifacting like the picture below before i overclocked it... BTW this is a brand new card that I sent back after the first one had artifacts. And I continued to get artifacts even after I got the new card, used a driver cleaner, and reinstalled different drivers.

Might be for couple of reasons, your old drivers might be messing up the current ones, thats if you didnt do a clean instal, also your psu might not be giving the juice that the card needs...

Do you have ATI tool? I always try to stress test for 15+ mins even if it's only a minor bump, helps to pick out instability that would otherwise go unnoticed.
Also, what are your case temps? How good is your airflow? Is the fan working properly on your card?

case temps are around 22 Celcius, the 9800 runs at about 60-70 Celcius, and the fan seems to be working properly.
